Why Google Reviews Matter More Than Most Physicians Realize

When a prospective patient searches for a physician on Google, the first thing they see — before your website, before your credentials, before any description of your services — is your star rating and review count. In a matter of seconds, that information shapes their first impression of your practice.

Reviews influence decisions in a way that is difficult to replicate through any other marketing channel. A well-written patient review communicates trust, competence, and the quality of the patient experience in language that feels genuine precisely because it comes from someone with no stake in promoting your practice. That authenticity is something no advertising copy can fully reproduce.

Despite this, many private practice physicians either have very few reviews or have stopped actively cultivating them — often because they are uncertain about how to ask without feeling uncomfortable, or because they are worried about doing something they should not. Those concerns are understandable and worth addressing directly.

What HIPAA Has to Do With Reviews

HIPAA does not prohibit you from asking patients for reviews. It does, however, govern how you respond to reviews — and it affects the tools you use to request them.

The most important HIPAA consideration in the review context is this: you cannot confirm or deny that someone is a patient. If a patient leaves a public review and you respond in a way that acknowledges their treatment, their condition, or any detail of their care, you have potentially disclosed PHI in a public forum — even if the patient initiated the conversation.

Responses to Google reviews should be warm and professional without engaging with any clinical specifics. Thank the reviewer for their feedback, invite them to contact your office directly if they have any concerns, and leave it there. This protects the practice regardless of the content of the review.

On the request side, the tool you use to send review requests matters. Automated review request platforms vary significantly in how they handle patient contact data. Any platform that stores or processes patient information — names, phone numbers, email addresses — in connection with healthcare visits qualifies as a business associate under HIPAA and requires a signed BAA. Verify this before deploying any automated review outreach system.

What the Medical Board and FTC Expect

Beyond HIPAA, two additional bodies of guidance are relevant to how physicians solicit reviews.

Most state medical boards prohibit the use of testimonials that are false, misleading, or that create unrealistic expectations about outcomes. While asking patients for honest reviews is generally permissible, any practice that selectively solicits reviews only from patients likely to leave positive feedback — or that offers incentives in exchange for reviews — is operating in ethically and legally questionable territory.

The Federal Trade Commission requires that any material connection between a reviewer and the business being reviewed be disclosed. Offering a discount, a gift, or any other benefit in exchange for a review without requiring disclosure of that relationship violates FTC guidelines. Google's own policies independently prohibit incentivized reviews. The practical guidance here is straightforward: do not offer anything in exchange for a review, and do not ask only the patients you are confident will respond positively.

The Right Way to Ask

The most effective review requests share two characteristics: they are timely and they are personal. A request that arrives shortly after a positive patient interaction — while the experience is still fresh — is far more likely to generate a response than one sent weeks later. And a request that feels like a genuine, human ask is more likely to motivate action than one that reads like a form letter.

Ask in person at the point of care. The most natural moment to request a review is at the end of a visit when a patient expresses satisfaction or appreciation. A brief, direct ask — something along the lines of letting the patient know that Google reviews help other people find the practice and that their feedback would be genuinely appreciated — is not presumptuous. Most patients who have had a good experience are happy to share it. They simply have not thought to do so unless prompted.

Follow up with a direct link. The friction involved in finding a business on Google and navigating to the review form is enough to prevent many well-intentioned patients from following through. Sending a follow-up message — whether by text or email — with a direct link to your Google review page removes that friction entirely. The link can be generated from your Google Business Profile and shared in whatever format your practice uses for patient communication.

Make the ask simple and pressure-free. The language of the request matters. Asking a patient to "leave a five-star review" is both ethically problematic and counterproductive — it signals that you are optimizing for a rating rather than seeking genuine feedback. Asking a patient to "share their experience" if they feel comfortable doing so is an invitation, not a demand. Patients respond better to the latter, and it is the only framing that is appropriate.

Train your front desk staff. For most practices, the front desk team has more direct patient interaction than the physician. Staff who are comfortable mentioning reviews as part of a natural end-of-visit conversation can significantly increase the volume of requests made without requiring the physician to personally raise it every time.

Automated Review Request Systems

For practices with higher patient volume, manual review requests are not always practical at scale. Automated systems can send review request messages triggered by appointment completion, which removes the reliance on individual staff members remembering to ask.

These systems can be effective when configured thoughtfully. The message should be brief, genuine in tone, and sent within a day or two of the appointment while the experience is still top of mind. It should include a direct link to the review platform. And it should not be sent repeatedly to the same patient — a single follow-up is appropriate; multiple reminders are not.

As noted above, the platform handling these requests must have a BAA in place if it is processing patient contact information in connection with healthcare visits. Verify this before deployment rather than after.

What to Do With Negative Reviews

A review request strategy that reaches a broad range of patients — rather than selectively targeting those most likely to be satisfied — will occasionally surface a negative review. This is not a reason to avoid asking. A small number of negative reviews among a larger body of positive ones is normal, expected, and actually increases the credibility of your overall rating. Profiles with uniformly perfect scores are viewed with more skepticism by prospective patients than profiles that show a realistic distribution of feedback.

When a negative review appears, respond promptly and professionally. Acknowledge that the patient's experience did not meet expectations, invite them to contact your office directly to discuss the matter, and keep clinical details entirely out of the response. Do not argue, do not defend, and do not disclose anything that could be read as confirming or describing the patient's care.
